First presented at the OPSO Annual CME Conference September 28th in Portland. OPSO is pleased to offer this full day program providing an intensive update in diabetes from a variety of perspectives. Join us for this 5.75 credit program exploring Prediabetes; Diabetes medications and improving patient adherence; Insulin utilation for the treatment of T2DM; Rational goal-setting for the treatment of diabetes in the elderly; Continuous glucose monitoring, and diabetes and pump technology.
Educational Objectives:
– Introduce the audience to the need for DM2 prevention.
– Review the criteria for pre-diabetes.
– Introduce the DPP (Diabetes Prevention Program) and the Oregon Health Authority/OMA Steering Toward Health program that uses the DPP.
– Encourage providers to refer patient who qualify to the DPP.
– Understand evidence-based recommendations for non-insulin therapies for optimal diabetes management
– Define 3 types of non-adherence and the difference between intentional and unintentional non-adherence
– List at least one solution for overcoming the most common barriers to diabetes medications
– Identify the challenges and barriers to diabetes care in the elderly
– Analyze and contrast the ADA-EASD and AACE algorithms for diabetes management, with focus on algorithms’ approach to the elderly where cited in these guidelines
– Interpret recent cardiovascular safety studies for diabetes agents
– Define goals and A1c targets for the elderly population
– Discuss the how and when in starting basal insulin in T2DM
– Explore ways to optimize insulin type and dose
– Use a case based approach to demonstrate best practices in insulin use in type 2 diabetes

Dr. Kevin Miller was born on Eglan Airforce Base in Florida, and grew up on many different bases in the mid-west and east coast. Inspired by his grandfather’s example of professionalism and caring, he chose to pursue medicine, graduating from Oral Roberts University in 1991, then going on to Kirksville College of Osteopathic Medicine where he did an additional year fellowship in Osteopathic Manipulative Medicine. After graduation in 1996, he completed a residency at Ohio University. “Being a physician allows me to serve people in need through science and compassionate care,” says Dr. Miller.
After his wife, Dr. Eden Miller, a native Oregonian, finished her training in Portland, she suggested the family look for an opportunity to move to Central Oregon and High Lakes provided that opportunity. “We have enjoyed seeing the wonderful people of Sisters for 10 years. The combination of great people, natural beauty and wonderful opportunity for raising healthy children have kept us enjoying our practice.”
Dr. Miller is a current board member of the Osteopathic Physicians and Surgeons Board. He is the past President of OPSO and American College of Osteopathic Family Physicians of Oregon and is also the current president of the American College of Osteopathic Family Physicians of Oregon. He has served as a delegate on the AOA board of delegates representing Oregon at this yearly meeting in Chicago. He was also serves on the Governor’s Board of Rural Health.
“I enjoy being involved in my children’s lives, going on family hikes and participating in their school and sports activities. I also serve on many boards in order to help improve the health and well-being of patients in our state and nationally.”
Teresa Martin, RD, CDE, LD Related seminars and products: 2
Teresa Martin is a mother of 3 children and lives in Bend, OR. She is also a Registered Dietitian with over 29 years of experience in the field of nutrition including clinical nutrition, community nutrition, diabetes research and most currently as a diabetes educator for Novo Nordisk. Over the last 16 years, Teresa has specialized in the field of diabetes and as a Certified Diabetes Educator; she helps individuals with diabetes gain the skills they need to better manage their diabetes so they can live the life they want to live and she helps healthcare professionals get the resources and training they need to provide quality, compassionate diabetes care. Teresa was recently recognized for her work by the American Association of Diabetes Educators of Oregon when she received the Diabetes Educator of the Year 2017 and Distinguished Service Award 2015. Teresa was an All-American Volleyball Player and All-American Alpine Ski Racer in College and continues to stay active playing competitive beach volleyball. Teresa values utilizing the outdoors, mindfulness, and the power of communities to help individuals find their path to a healthier, happier life.
Jay Shubrook, DO Related seminars and products: 2
Dr. Shubrook’s professional work and passion focuses on preparing the primary care workforce to better manage diabetes and optimizing the prevention and treatment of diabetes. After 10 years in primary care, he completed a Diabetes Management Fellowship and has been focusing on care of the patient with diabetes ever since. He served as the Director of an integrated comprehensive diabetes care center and also as the Director of Clinical Research at both Ohio University Heritage College of Osteopathic Medicine (HCOM) and, more recently, Touro University California College of Osteopathic Medicine. He served a the Director of the Diabetes Fellowship Program for Primary Care Physicians at HCOM and looks forward to expanding this training opportunity to new professionals and a new institution. Dr. Shubrook still advises the program at HCOM and serves on a national ad hoc committee of American Association of Clinical Endocrinologists on implementing primary care diabetes training even more broadly. He has served in a number of national roles such as Chair of the Primary Care Advisory Group of the American Diabetes Association and the Chair of the Research Committee of the Guidelines Advantage, collaboration between the American Heart Association, American Cancer Society and the American Diabetes Association. Dr. Shubrook has been NCQA certified throughout his time as a Diabetologist and has helped each of our Fellows achieve this honor.
Having served in both primary care and specialist roles, he is acutely aware of the gaps in the health care system that lead to fragmented and poorly-coordinated and executed care. Further, having been active in measuring quality of care, he is accustomed and very knowledgeable of the clinical shortcomings of clinical practice self-assessment.
Michael Shannon, MD Related seminars and products: 2
Michael H. Shannon, MD, is an endocrinologist and Medical Director with Physicians of Southwest Washington, an Independent Practice Association managing Medicare Advantage and ACO contracts, in Olympia, Washington. He is a Clinical Assistant Professor in the Department of Medicine at the University of Washington. He serves on the Editorial Board of Clinical Diabetes, and he is the Chair of the Professional Education Committee for the Washington chapter of the American Diabetes Association.
He served as the System Endocrinology Lead and Chair of the Endocrine/Diabetes Clinical Advancement Team for Providence St. Joseph Health, a 51-hospital integrated health system, from 2011-2018. He served as the Medical Director for his region’s Accountable Care Network, dedicated to quality metrics and reductions in the total cost of care.
Dr. Shannon graduated from medical school at the University of California, San Diego, and then he went on to perform an internal medicine residency, geriatrics fellowship, and endocrinology fellowship at the University of Washington.

High Lakes Health Care
Dr. Eden Miller, an Osteopathic board certified Family Practitioner, received her medical degree from Kirksville College of Osteopathic Medicine in 1997. Returning to the Northwest, she completed her residency in family practice at Eastmoreland Hospital in Portland, Oregon. Upon completion, she joined High Lakes Heath Care in Bend, Oregon, where she shares a practice with husband Dr. Kevin Miller.
Over the past 15 years, Dr. Miller has cultivated a special interest in diabetes, after contracting type-1 diabetes while in medical school. Out of that personal experience, her practice has extended into a subspecialty in diabetes care. An impassioned speaker, Dr. Miller has given over 600 lectures in the field of diabetes to health care providers and patients alike, on subjects such as Diabetes Management, Insulin Pump Therapy, Clinical Research, and Healthcare Policy. Her adage for care is that she “only succeeds as a provider if she turns patients into experts on their own disease”. Dr. Miller believes that a physiologic patient individualized approach to diabetes intervention is crucial to success. She feels if our nation is to triumph over the ravages of diabetes, a collaborative effort; unhindered by fear of personal scrutiny, and barriers for effective treatment need to be removed. For her, Diabetes Nation provides the foundational model to impact the people that represent this disease, and begin the process of restoring hope back to their lives. Dr Miller and her husband keep busy with their two children enjoying the wonderful central Oregon outdoors while snowboarding, boating and playing basketball.
